Heritage Park of North Iowa 2006 Raffle Forms

To enter a raffle, print this page then send the appropriate completed form(s) and your remittance to:

Tickets are $1.00 each or 6 for $5.00. If you purchase more then one chance, you must fill out a form for each chance. You can mix and match if you want. For example if you buy 6 tickets for $5.00 you can fill out 3 forms for Raffle 1 and 6 forms for Raffle 2.

The drawings will be held approximately 5:00 PM on Sunday September 17th, 2006. Only properly completed forms with correct remittance received by Saturday September 16th, 2006 will be included in the drawing.
